Immune reconstitution inflammatory syndrome in AIDS patient after successful induction of virological suppression with cabotegravir/rilpivirine

چکیده

Long-acting (LA) cabotegravir/rilpivirine (CAB/RPV) is a complete regimen for the management of human immunodeficiency virus type 1 (HIV-1) infection to replace their oral antiretroviral therapy (ART) when they have been virologically suppressed. We present case successful achievement undetectable HIV RNA viral load levels in an acquired syndrome (AIDS) patient with long-standing virologic failure within two months CAB/RPV LA initiation. This was later complicated by immune reconstitution inflammatory (IRIS) due Mycobacterium avium-intracellulare (MAI) and hepatitis B (HBV) reactivation.

Immune Reconstitution Syndrome in a Patient with AIDS

A 43-year-old woman with AIDS presented with hearing loss, dysarthria and ataxic gait with a history of 3 days. The patient had been re-started on antiretroviral therapy three weeks prior. Brain magnetic resonance imaging (MRI) was performed (Picture). A JC virus polymerase chain reaction of the cerebrospinal fluid subsequently returned positive findings. JC virus is the etiologic agent of prog...

Neuroimaging Features of Immune Reconstitution Inflammatory Syndrome in a Patient with AIDS Successfully Treated for Neurocryptococcosis

The use of highly active antiretroviral therapy (HAART) has significantly reduced the incidence and progression of HIV-associated cryptococcosis. However, an early complication of HAART is the immune reconstitution inflammatory syndrome (IRIS), which may affect the CNS. The authors report a patient successfully treated for cryptococcosis and HIV who presented a late manifestation of IRIS. Neuro...

Gastrointestinal cryptococcoma – Immune reconstitution inflammatory syndrome or cryptococcal relapse in a patient with AIDS?

The introduction of antiretroviral therapy (ART) may lead to unusual paradoxical and unmasking presentations of opportunistic infections. Intra-abdominal cryptococcosis is a rare manifestation of Cryptococcus. We present the case of an HIV-infected patient on ART, with a history of cryptococcal meningitis who presented with subacute, worsening abdominal pain during immune recovery. This evolved...

Leprosy and Kaposi sarcoma presenting as an immune reconstitution inflammatory syndrome in a patient with AIDS.

The simultaneous presence of infectious organisms within cutaneous lesions of Kaposi sarcoma in persons with AIDS has been demonstrated. We describe a patient with concurrent leprosy and Kaposi sarcoma presenting as an immune reconstitution inflammatory syndrome in the setting of AIDS.
